When I made my cardboard piecepack in 2004, I used a craft hammer and circular die cutter from Sizzix (http://www.sizzix.com/).
I also bought blank 16mm cubes, one each in red, green, blue and black, from Great Hall Games (http://greathallgames.com/aacc/adice/adiceBlank.html) and turned them into piecepack dice.
